Clubhouse Community Support Program.

The Clubhouse is a “One-Stop Shop”, functioning as a community-based hub - a single point for people to access, integrate, and coordinate a range of services - to work toward mental health recovery.

WHAT IS THE CLUBHOUSE MODEL?

Centered on relationship building and community, a Clubhouse is comprised of “members” and staff working together side-by-side as a community to operate the Clubhouse, link members to needed services, provide social engagement, and share decision making.

We offer peer support to help people with mental illnesses work on their goals in employment, volunteering, health and wellness education. And let’s not forget F-U-N! Vail Place Clubhouses offer a wide variety of social, recreational, and cultural activities.

The Clubhouses are enrolling new members right now!

HOW TO BECOME A MEMBER

You must be over age 18, live with a serious mental illness and be able to attend the Clubhouse independently. Membership is open to those living in our Twin Cities Metro Service Area, with or without insurance coverage and/or waivers. You do not need a referral from a mental health provider to begin the process. And Clubhouse membership is FREE!

  • A one-hour appointment is set up to review Clubhouse Enrollment details.
  • Select an activity for your First Day with the Clubhouse community.

Case Management.

PROGRAM GOALS

To provide the assessment, monitoring, planning, advocacy and connection to supports and resources to help you achieve your recovery goals and live independently.

PROGRAM DESCRIPTION

A Case Manager can work with you to help you get connected to the services you want and need to support your independence, health and recovery.

Vail Place Case Managers:


  • Engage in services collaboratively with you, your providers, family, and natural community supports.
  • Help you identify goals and develop strategies to achieve these goals.
  • Complete assessments outlining your strengths, needs, and goals.
  • Meet with you in your natural environment – home, community, medical/psychiatric appointments, etc.
  • Develop and maintain relationships with many providers and agencies so they can effectively provide you with referrals and coordinate your care and needs.

Our Case Management teams include case managers, social workers, nurses, chemical health specialists, vocational staff, housing staff and clinical supervisors.

We employ individuals who speak Amharic, Arabic, Somali, Spanish and American Sign Language and partner with an external agency to provide language interpretation services.

SHORT-TERM
HELP

Vail Connect.

PROGRAM GOALS

To stabilize clients and improve health outcomes in order to connect them to long-term primary and psychiatric care and community supports.

PROGRAM DESCRIPTION

This is a mobile, short-term service, typically lasting no longer than three months. Our Navigators meet you in the community or your home to help connect you to services and supports.

IMPORTANT NOTE: This service is limited to individuals connected to North Memorial Health (NMH) and affiliated clinics.

A Vail Connect Navigator can help you, your family or involved care providers in obtaining:

  • mental and/or physical health assessment
  • treatment
  • resources
  • support

HELP BEFORE
CRISIS STRIKES

Vail Care:
Behavioral Health Home.

PROGRAM GOALS

To help people with complex mental and physical health needs set goals and strategies to manage their overall health.

PROGRAM DESCRIPTION

Vail Care is a service provided by a team of individuals – a nurse and others who help you in all areas of your life. The team members assist you in getting medical and mental health services and finding social and community supports. Staff work with you in your home and in the community and help you create a Health Action Plan, provide coaching and support to help you in meet your health goals.

Vail Care is a Medicaid-certified behavioral health home services program. It includes:

  • health and wellness promotion
  • case management
  • enrollee and family support
  • care management, and
  • referrals for community-based services

HELP WITH
SAFE HOUSING

Vail Place Housing Programs.

PROGRAM GOALS

Preparing Vail Place members for independence.

PROGRAM DESCRIPTION

Vail Place Housing Programs address the comprehensive needs of people who are homeless and living with mental illness and chemical dependency. We emphasize combating homelessness through our two newest programs – Rapid Rehousing (RRH) and Housing Stabilization Services (HSS). RRH exclusively supports those who are exiting homeless shelters or living on the streets. HSS is a statewide initiative that works with those that are at risk of homelessness and strives to find alternative housing or prevent loss of current housing.

Vail House, our 23-bed group residential housing (GRH) facility is in SE Minneapolis and provides transitional housing and support services in a healing environment.

Vail in the Park, our Louisiana Court Apartment Complex in St. Louis Park supports individuals that meet the state’s “long-term homeless” definition, helping residents learn the skills to be successful renters and avoid future instances of homelessness.

COMPREHENSIVE
HELP

Assertive Outreach Program.

Assertive Outreach and Stabilization (AOS) is an integrated care model designed by Vail Place to engage individuals in crisis experiencing mental illness, chronic substance abuse disorders, and a variety of social and health-related consequences.

The goal is to help stabilize people in the community and ultimately reduce healthcare costs. Over the past year we have doubled our staff including the addition of one dedicated RN to meet the needs of individualized expanding partnerships.
