Community and Health: Stories of Impact and Collaboration, Nov. 17

On Tuesday, November 17 from 6-8pm Vail Place will host a community conversation, “Community and Health: Stories of Impact and Collaboration.” Vail Place and the State Innovation Model (SIM) present this story-sharing event that will feature patients, staff, and other partners’ personal experiences and impact of collaborative care. The stories will showcase person-centered care, care coordination and emerging health professions. An interactive discussion will explore the connection between social determinants of health, community resources and partnerships, and patients’ experiences of care.

From 6-7pm a panel will present the program, followed by a Q&A session from 7-7:30pm. A desert reception will be held from 7:30-8pm.

The panel will feature:

  • Event Facilitator – Claire Wilson, Executive Director, Minnesota Association of Community Mental Health Programs
  • Family Member and Mental Health Advocate – Nancy Howe on parenting a child with a mental illness, chronic health concerns, and multiple providers
  • Portico Community Health Worker – Mai-Eng Lee on access to care
  • Community Paramedic – Pete Carlson on filling the gap in mental and health care services
  • North Memorial Care Coordinator – Rebecca Nixon, Clinic Care Coordinator, Advanced Care Planner facilitator, RMA, X-Ray Tech. on the continuum of care in healthcare
  • Vail Place Case Manager – Carolyn Bollinger on advocating for the ‘whole person’ and their needs
  • Vail Place member – Julie T. on life with a mental illness and navigating gaps in services
